There were no paper towels at the hand wash sink. Napkins were brought out. Please provide paper towels before the next meal service and ensure the hand sink always has hot running water (100-120 F), soap and paper towels. (10 red)

El Amigo Stationary Mobile, Centralia 20/0/20

The hand sink was completely blocked by a large bus tub and other items. Paper towel dispenser was empty and backup paper towels were not easily accessible. These were corrected. Please ensure that the hand sink is always clear, accessible and fully stocked with hot running water (100-120 F), soap and paper towels. (10 red)

Several potentially hazardous foods were observed cold holding between 43 and 54.2 F and must be kept at 41 F or below at all times. Based on time and temperature, these were placed in the freezer to cool. (10 red)

Pork carnitas were hot holding between 120 and 124 F and must be kept at 135 F or above at all times. Because they were recently cooked, they were reheated to 165 F. Please monitor frequently to ensure safe temperatures. (25 red)

Potentially hazardous foods were cold holding at approximately 46.7 F and must be kept at 41 F or below at all times. Foods were uncovered and refrigerator was adjusted. Please monitor temperatures very closely and discontinue using this refrigerator for PHFs if they cannot be kept at 41 F or below. (10 red)

One of the refrigerators was not working properly and several containers of tomatoes, lettuce and cheese were cold holding between 45 and 46 F. All potentially hazardous foods must be held at 41 F or below during refrigeration. These were all discarded and refrigerator will be adjusted as needed. (10 red)

Hot dog bun was served after preparing with bare hands. Bare-hand contact is not allowed with any ready-to-eat foods. Please use gloves or utensils as needed. (25 red)

Bleach solution is too weak (10-20 ppm). Please adjust to 50-100 ppm and use test strips to test when prepared and then at least every two hours. (5 blue)

Person in charge (initially) did not have a valid food worker card available. Another employee with a valid card arrived. (5 red)

The employee bathroom does not meet food code requirements at this time. The toilet has a leak and can only be flushed by turning on a valve and then turning it off again. (3 blue)

A small open-air cooler was not maintaining the required temperature and milks were cold holding at 50.4 F. All potentially hazardous foods and beverages must be kept at 41 F or below. These were discarded and refrigerators will be adjusted and monitored frequently. This is a repeat violation. (10 red)

There were dirty dishes in the hand was sink. Please keep sink clear at all times. Dishes were removed. (10 red)

There was no sanitizer solution available. Seventy percent isopropyl alcohol has been used to sanitize the tortilla conveyor belt. Please stop doing that and instead use a 50-100 ppm bleach solution. Note: Test strips were found. (5 blue)
